About MDT

Henrico County Public Schools is committed to making decisions that result in positive outcomes for students.  Initial decisions involving students with disabilities should be made at the school level.  These decisions may include eligibility to receive exceptional education and related services, class schedules, and the amount of exceptional education and related services provided.  Initial decisions are typically made by a team of professionals within the school including school counselors, school psychologists, social workers, general and special education staff, school nurses, and related services staff.

After meetings with teachers, parents, administrators and other educational professionals, some students may continue to present challenges that negatively affect their social, emotional and educational growth.  Therefore, presenting information before MDT may be necessary.

How present a case to the Multi-Disciplinary Team

  1. Discuss the case with school-based professionals and parents.
  2. In cases involving behavioral issues, document and implement school-based interventions such as Behavior Intervention Plans (BIP), and collect data on the outcome of those interventions.
  3. Consult with the exceptional education coordinator or specialist for your zone.
  4. Complete the MDT referral form.
  5. Download and complete the CSA Authorization and Release form (parent signature required).  This form allows members of the MDT to discuss confidential and sensitive information.
  6. Return completed forms to the Exceptional Education Specialist or Coordinator for your zone.
  7. Once a meeting is scheduled, a representative from the school team should attend and participate.

After the meeting the Multi-Disciplinary Team may:

  1. Suggest additional interventions or supports to be provided in the current educational placement.
  2. Suggest additional education and/or related services to be considered by the IEP team.
  3. Provide information for supports offered by Henrico Mental Health, Henrico Social Services or Henrico Courts agencies
