Incident Reports

Reminder: You must complete the attached Incident Report any time you experience any of the situations listed in section 8 of this form. The form that is attached, can also be found in the “Quick Reference” folder in your school’s “Safety” folder on Google Docs. It only takes a minute to complete the required form and submit it to schoolsafety@henrico.k12.va.us. Please call the School Safety Office at 652-3511 if you have any questions.
